• All Family Living Optometrist network members pay strictly cash only. Members can only claim consultation fees from medical aids directly (depending on medical aid terms and conditions).
• Upon consultation membership card or member statement and member identity document is required to receive Family Living Optometrist Network 10% consultation discount and 10% frame benefit.
• Appointment at Optometrist practise must be made by either a Family Living Optometrist Network call centre agent or by the Family Living Optometrist Network member.
• Family Living is granted the right to add branding / marketing material, within reasonable bounds, to the optometrist’s consultation rooms. Optometrists will be responsible for the implementation and maintenance of Family Living marketing material, in case of loss or damage, optometrists are requested to contact Family Living to replace such items.
• Optometrists agree to bill regular consultation fees minus the
10% consultation discount and 10% frame discount granted to Family Living Optometrist Network principal and dependant members.
